Advance Directives Workshop

Many of us, it's estimated 90%, someday will face difficult medical choices.  ProHealth Care is offering this Advance Directives Workshop in an attempt to help you prepare for that time in your life and/or the lives of your loved ones.  Kerry Case, M.D., M.S., ProHealth Care's Palliative Medicine specialist, will lead this workshop, including a step-by-step guide to understanding, creating and using advance directives.  Blank templates of the forms used in Wisconsin will be available at the workshop.  Please register in advance.
