Aurora Health Care Center Receives High Recognition

Waukesha clinic obtains Level 3 recognition as a patient centered medical home.

An Aurora Health Care center in Waukesha has received a Level 3 recognition – the highest available - as a patient centered medical home from the National Committee for Quality Assurance (NCQA), according to a news release from the medical group.

From the news release:

There are now 57 Aurora sites that have achieved Patient Center Medical Home recognition within the Aurora system, which was the first health system in Wisconsin to receive the Patient Centered Medical Home recognition and designation.

  • Aurora Health Center in Ripon
  • Aurora Health Center in Twin Lakes
  • Aurora Health Center in Walworth
  • Aurora Health Center in Waukesha
  • Aurora Sheboygan Clinic
  • Aurora Health Center in Caledonia
  • Aurora St. Luke's Health Center in Franklin
  • Aurora Health Center in Kenosha

The Patient Centered Medical Home recognition acknowledges a health care setting that facilitates partnerships between individual patients, their personal physicians, and when appropriate, the patient’s family.
